Password Request Form

We have been having some difficulty recently with Web site users receiving the Lost Password messages. It's possible that, since those messages are auto-generated, they are getting caught in a spam filter or something. If you have submitted a Lost Password request and have not received e-mail notification, please call (800) 968-6004 between 8 and 5 EST M-F and follow the prompts for Tech Support. Let them know you need help with an Illinois State user password problem.
